There are in excess of 160,000 dentists in the United States today. The American Dental Association and the National Association of Dental Schools has placed a priority on the application of diagnostics to be.used by dental practitioners. The focus is to be proactive rather than reactive. Today, dentists treat disease and problems, and apply preventive care. In the medical profession, it is normal for the patient to seek regular physicals to avoid medical problems. In the dental field, diagnostic testing now provides the dentist with the tools to provide similar services so as to provide the patient with a plan to avoid dental problems well in advance of any visible onset.

Depending on how well the patient may be expected to tolerate concomitant antibiotics, a sequential approach may also be considered with Amoxicillin or Augmentin first. In order of increasing efficiency and potential side-effects, the antimicrobials could be ranked as follows: the tetracyclines, clindamycin, amoxicillin, and metronidazole. This ranking is somewhat subjective and individual results may vary. It would be prudent to correlate your laboratory results with the patient's clinical presentation.
In addition to clinical presentation, order considerations include patient compliance with the regimen: should an oral route be considered over sub gingival placement or vise versa.
If choosing an oral regimen, the next issue is to consider single or combination therapy. Also, it is important to check the PDR or other references to be familiar with inherent side effects, drug interactions, and other potential problems when considering an antimicrobial.
Single Therapy Options
- Tetracycline - according to the literature, antibiotic resistance may develop if the drug is prescribed for only two weeks. In light to this potential problem, I typically place patients on a three-week course. 100 mg of Tetracycline are equivalent to 100 mg of Doxycycline or Minocycline. Since dosing would be easier and compliance better, Doxycycline or Minocycline might be a better choice. Also, in site-specific disease, controlled local delivery of tetracycline may be a better choice all together, when dosing is not an issue.
- Clindamycin - 150 mg quid for ten days
- Amoxicilln - There are two choices, plain Amoxicillin 250 mg of 500 mg tid for ten days, or Augmentin (Amoxicillin plus Clavulanic Acid) 250 mg or 500 tid for ten days. Augmentin is indicated for the more aggressive cases.
- Metronidazole - 250 mg tid for ten days.
Combination Therapy Options
- Doxycycline followed by Metronidazole - three week course of Doxycycline then a ten day course of Metronidazole.
- Amoxicillin with concomitant Metronidazole - both for ten days (consider lower dosages first, depending on aggressiveness of disease)
Augmentin with concomitant Metronidazole - both for ten days (consider lower dosages first, depending on aggressiveness of disease)
